Our skilled nurses and clinicians are experts in working with all types of patients—from the elderly, to those with chronic physical or psychiatric conditions. We are passionate about meeting their individual needs and providing an alternative to inpatient hospitalization.

Making a referral is fast and easy.

We welcome referrals 24 hours a day, 7 days a week—and usually can have care in place within a few hours. During business hours, our Intake Representatives can be reached at 800.915.3211. CONCERNED ABOUT LANGUAGE ISSUES?

Nizhoni employs clinicians and nurses who are fluent in Spanish, Portuguese, Haitian/Creole, and 23 other languages. In addition, our language line service enables any language to be translated from patient to nurse.
