Nizhoni Health Systems

Helping patients return home and remain at home.

At Nizhoni Health, we provide patients with the comprehensive care to support their recovery and health, in an in-home setting.

From temporary wound care, to ongoing medication monitoring, to the management of complex, chronic, medical or psychiatric conditions, we go above and beyond to deliver the highest possible quality of home-based health care and support.

Improving outcomes, reducing readmissions, controlling costs.

Our skilled nurses and clinicians are experts in working with all types of patients—from the elderly, to those with chronic physical or psychiatric conditions. We are passionate about meeting their individual needs and providing an alternative to inpatient hospitalization.

To accomplish this, we:

  • Assess the specific needs of each patient in their home setting
  • Identify potential concerns
  • Coordinate care with a patient’s existing medical providers
  • Assure the patient has the right combination of support to meet their needs – from activities of daily living to more complex nursing care
  • Provide access to caregivers
